Sage Midterm Check-In Progress Survey

On Oct. 9, faculty should receive an invitation to complete a Progress Survey from the Office of Instruction with an email subject: SAGE: Student Progress Survey. This is completely voluntary, but a great way to provide and express concerns or encouragement.

To start, select one of your current courses from the drop-down menu, and open the survey for your students.

Next to each student, you’ll see 4 checkboxes. If you have any concerns about that student, or would like to provide positive feedback, you can use these checkbox options to quickly notify the student and other campus support services as appropriate. Examples are provided below.

Concerns will be sent to the student and their advisor. Serious concerns (e.g.: failing a course, being consistently absent, or experiencing personal issues) may also be submitted directly to Student Care and Outreach via a CARE Report.

Tier 1 Concern

SAGE sends a message to the selected student that includes any comments you add. A Tier 1 concern might be appropriate if the student

  • has missed a few class sessions
  • has missed an assignment deadline, or
  • performed poorly on an assignment.

Tier 2 Concern

SAGE sends a message to the selected student AND the student’s academic advisor with faculty-entered comments. If the student doesn’t respond, the advisor will follow up with them. A Tier 2 concern might be appropriate if the student

  • has missed more than a week’s worth of class sessions
  • has missed several assignment deadlines
  • performed poorly on multiple assignments.

Tier 3 Concern

SAGE sends a message to the selected student, student’s academic advisor, AND the Office of Student Care and Outreach with faculty-entered comments; SCO follows up with the student if there’s no response. A Tier 3 concern might be appropriate if

  • the student has been consistently absent
  • never turned in an assignment
  • is failing the class, and/or
  • you suspect they may be experiencing personal issues that are impacting their ability to be successful in class (financial, mental/physical health, family, etc.).

Keep Up the Good Work Kudos

SAGE sends a general message of encouragement directly to the student, including any faculty-submitted comments.

NOTE:

If you do not have feedback for some students in your course, just leave all boxes un-checked for those students.

Once you finish, press the “submit” button so your answers are recorded. Even if you have selected no items for your students, please still hit “submit” so that we know you have viewed the survey and have nothing to report at this time.
